Funding Request Forms

FUNDING REQUEST FORMS

GMS faculty and staff may submit funding requests to the GMF by completing the designated request form and forwarding it to governormoreheadfoundation@gmail.com for review. Please ensure that Principal Daniel Simmons is copied on all submissions to facilitate coordination.

All requests must be submitted at least two (2) weeks in advance of the anticipated need. Submissions received with less than two weeks’ notice may not be reviewed or approved. For more complex requests, earlier submission is strongly encouraged to allow sufficient time for follow-up questions and, if necessary, the scheduling of a meeting to obtain additional clarification beyond the information provided in the form.

For requests involving the purchase of items, the Board requires a detailed, itemized list including pricing and images. Additionally, please include a clear justification for the selected items, particularly where specific choices have been made over alternatives, outlining their suitability and intended purpose.

FOLLOW UP: It is important that after your item is used, you must:

  • Return any unused funds to the GMF Treasurer, Mike Perkins
  • Submit all proof of purchase documentation (receipts, invoices, etc.) to the GMF Treasurer, Mike Perkins
  • Provide photographs, articles, or videos of the event or installed items to the GMF email for use by the GMF Public Relations team

Providing thorough and detailed information in your request will greatly assist the GMF in its review and decision-making process.

Included below is a copy of the blank Request Form, along with examples illustrating the level of detail expected for requests involving tangible items (e.g., furniture, décor, etc.).

Example: A floor plan indicating the location and layout of requested items. This demonstrates intent, quantity, and spatial compatibility, and assists the Board in visualizing the proposed use of the space.

Example: Product documentation including images and pricing. This supports evaluation of durability, quality, aesthetics, and overall value.

Please provide images and pricing for all items included in your request, including smaller components (e.g., planters), where relevant. Routine consumables (e.g., paper, pens) do not require the same level of detail unless they are central to the request.
